Do you know a young adult between ages 16-21 who you think would benefit from our program?
Please have the young adult fill out the intake form below and bring it into the center, or send your young adult to the center and we will help them fill it out. Insurance, IDs, or referrals are not necessary to be a part of our program. Our services are completely free. Contact us via phone or email to schedule a time to come into the center.
